Specific Use Permit to allow an alcohol and drug
an RM-H (Residential Multiple Family Dwelling —
Shalom Health Care Center has requested a Specific Use Permit to allow an alcohol and drug
rehabilitation facility at 950 Washington Boulevard. More than half of the property is zoned
GC -MD (General Commercial - Multiple Family Dwelling). However, the portion of the
property where the alcohol and drug rehabilitation facility will be located is zoned RM-H
(Residential Multiple Family Dwelling - Highest Density). The applicants state that the facility
will begin by only treating adult female patients, but that adult males would not be denied.
Patients will remain at the facility for 15-20 days. The facility will have 8 beds available for
detoxification and 16 beds available for residential past -detoxification treatment. In addition, the
facility will offer outpatient programs for alcohol and drug abuse.
Security will be provided by an eight foot fence around the entire facility along with 24 hour on
campus security guards. Security cameras will also be used. Patients will not be allowed outside
of the buildings during treatment.
Staff recommends approval of the request with the following condition:
1. Construction plans must meet all requirements by Water Utilities for water and sanitary sewer
services, including the requirement of the City's pre-treatment program.
Please note that final occupancy approval is subject to review and acceptance of submitted plans
and field inspections to verify compliance with applicable codes.

Dear Ms. Josey,
I, on behalf of the Board of Directors of Shalom Health Care Center and on behalf of
physicians, nursing staff, counselors, and behavioral health expert; would like to express
my profound gratitude and utmost thanks for your leadership and commitment to the
community development and towards positive city planning services. Special thanks to
all those who provided their insight, guide us and walk through to understand this
application process.
The Shalom Health Care Center has a vision to establish comprehensive medical clinics
that served underserved areas of Jefferson, Hardin, Orange, Newton, Jasper and liberty
Counties. Our mission is to provide holistic behavioral and mental health services,
including outpatient, residential and hospital -based services. We are projecting that our
property located at 950 Washington Blvd. Beaumont, Texas will be the medical complex
that create more than 170 new jobs, 24 hours residential care and 4o bedded facility (24
hours residential and 16 hospital -based patients.) This new medical complex will provide
access to unavailable behavioral and mental health services in the area.
Background
Addiction is a complex disease: It deteriorates human brain performance, thereby
impairing the ability to make sound judgments and decisions, which then leads to
character deterioration and can eventually cause total self-destruction. Addiction not only
impacts the individual; it also disrupts their surroundings, families and community at
large. As troubling as that sounds, this disease is treatable, and recovery is possible if
effective interventions are put in place to address sustainable recovery.

SHALOM HEALTH CARE CENTERS
Substance and Alcohol addiction kills 115 adults daily; which is more than 42,000
Americans each year. This number is six times more than what American army had lost
soldiers in Iraq and Afghanistan war. Consequently, the President of the United States
has declared addiction epidemic and will use all state and national resources to control,
prevent and treat substance use addiction.
Strategies for dealing with drug addiction risks should be designed to offer a
comprehensive program that starts with preventive education and goes all the way to
cover detox, recovery, post recovery education and sober living for those who are
addicted.
Shalom Health Care will provide preventive education and public awareness about
substance use and its social and economic implications on individuals and families. A
comprehensive post rehab program will be developed to ensure that those treated
returning home are fully supported on a sustained basis by the local community before
entering a productive life without any communal stigma or isolation. Additional addiction
treatment centers are needed to provide detox and rehabilitation services in the Southeast
Texas region.
Shalom Health Care Center is requesting the Planning and Development Department to
grant us Specific Use Permit to provide us comprehensive medical clinic, drug rehab
outpatient and residential recovery center in the buildings located at 950 Washington
Blvd. Beaumont Texas. Following are the highlights:
SUP — Request Highlights
Master Plan -of the property is attached. Currently 950 Washington Blvd, has 5
buildings. They are designated as 950/1 to 950/5 Buildings:
Only 950/3 & 4 buildings will require Special Use Permit for the purposes to establish
medical clinic, outpatient drug rehab and recovery programs. The 950/3 building
(Former UTMB — H shaped Building) which is almost 8,000 SF will be used for
comprehensive medical clinic and outpatient drug rehab services. However, 950/4
building; covered area = 6,304 SF will be used for inpatient residential care.
• These buildings will be used to treat substance use and alcohol abuse patients in
outpatient and residential care setting. The Facilities will be licensed by the Health
and Human Services commission for the treatment of addiction related patients. The
Licensed facilities will accommodate 24 Beds for Detox and Residential treatment.

• Patients will stay overnight from 3-5 days in Detox and 12-15 days in follow up
residential care. Patients will remain in the facility till they are fully recovered. It is
expected that in the first phase only female patients will be admitted, however, adult
males will not be denied. Patient ages 18 and above will be admitted. There will be no
persons from the Department of Justice or incarceration will be admitted.
• Outpatient Treatment Program; patients in need of alcohol and substance use treatment
will find the tools and support they need to turn their lives around. Medical clinic and
outpatient drug rehab will operate in 950/3 Building. (SUP Requested Site).
• Shalom Health Care Center Outpatient Services recognize that everyone comes to recovery
under unique circumstances and that most patients must also keep up with work,
schooling, and family responsibilities; will be facilitated by a Licensed Chemical
Dependency Counselors and professional physicians. Integrated medical, behavioral
and mental health center will be established to address needs of underserved in the
region. Medical Clinic, Drug Rehab outpatient center will be the entry point for
residential inpatient drug rehab program.
• Inpatient Residential; Shalom Health Care will offer inpatient 8-bed detox in our
residential facility Building 950/4 (SUP Requested Site). In addition to that 16-bed
residential post detox treatment facility will provide comfortable, home -like setting,
onsite. Our medical team will provide inpatient detox services. We will provide
around -the -clock monitoring and use the latest evidence -based medications.
• SubstanceAbuseAssessment:; For those who may be developing a chemical dependency,
early assessment can highly warning signs before more serious problems develop. For
others, a professional substance use and alcohol assessment may be the first step
towards effective treatment recovery.
• The facility will provide counseling services, medical exams, group therapies,
recovery, rehabilitation and family support services. Whereas, patients will have
dining area, bathrooms, showers, laundry space and family waiting areas.
• More than 6o people will be employed for direct patient care in the residential setting,
however, more than 50 individuals indirectly will be benefitted which including
transportation, security, catering, pharmacy, delivery, washing and laundry, yard
maintenance, building maintenance, computer personnel, marketing, medical coding
and recording services.
• Security Measures; the facility will be 8 foot fenced, 24 hours security guards will be
in the campus, CCTV cameras will be in place, and security post will allow in and out
of permitted individuals. Building is completely cordoned with fences for patient's

safety. Patients will not be allowed to leave the building during treatment procedure
and services till fully recovered. Due to privacy reasons patients will not be seen by the
community or neighbors.
We are requesting the Planning and Community Development Department to grant
Shalom Health Care Center Special Use Permit for the Buildings 3 & 4 located at 950
Washington Blvd. to operate outpatient medical clinic, drug rehab and inpatient
residential drug recovery program. All other buildings are according to zoning code
requirements.

1. That the specific use will be to provide inpatient residential treatment services to
the substance use and alcoholic abuse patients. This facility will not accept
inmates or persons from the Department of Justice. This building will not provide
any enjoyment or entertainment in the facility. This facility will exclusively be
used for medical purposes and specifically for inpatient residential recovery
services with authorized access. This facility will have all CC TV Camera and open
areas will be fenced. Access will be allowed through security measures.
2. Because this facility will be the part of Shalom Health Care Center's medical
complex. The inpatient residential facility will have confined public access. This
facility will increase the value of the surrounding properties.
3. Necessary road, utility, drainage and other necessary facilities already exist. No
other development is needed.
4. Since this facility was previously operated by the City of Beaumont, Health
Department and the University of Texas Medical Branch, facility is completely
built according to ADA complaint with enough parking spaces with easy access
and driveways. No further construction.or improvement is needed.
5. There will be no nuisance, offensive odor, fumes, dust, noise and vibration. This
will be in fact; quiet, clean and supervise facility.
6. All directions signs will be installed, few parking lights already exist, however, if
needed will be installing according to city code compliance.
7. Sufficient land scape already exists, all green spaces lawns are in good shape.
