BE IT RESOLVED BY THE CITY COUNCIL
OF THE CITY OF BEAUMONT:
THAT the City Manager be and he is hereby authorized to execute a Memorandum of
Agreement between the City of Beaumont and Baptist Hospitals of Southeast Texas
d/b/a Baptist Hospital to provide laboratory services to screen for HIV and Hepatitis C
(HCV) from specimens collected by the City of Beaumont Health Department to ensure
that adults in the City of Beaumont are properly diagnosed and treated for these two
diseases. The Memorandum of Understanding is substantially in the form attached
hereto as Exhibit "A" and made a part hereof for all purposes.

MEMORANDUM OF AGREEMENT FOR
LABORATORY SERVICES
BY AND BETWEEN
THE CITY OF BEAUMONT HEALTH DEPARTMENT AND
BAPTIST HOSPITALS OF SOUTHEAST TEXAS d/b/a
BAPTIST BEAUMONT HOSPITAL
This Memorandum of Agreement for Laboratory Services, is made and entered into by and between City of
Beaumont Health Department ("The City") and Baptist Hospitals of Southeast Texas d/b/a Baptist Beaumont
Hospital ("The Hospital") with an effective date of May 1, 2017 ("Effective Date").

The purpose of this agreement is for the Hospital to provide laboratory services to screen for HIV and
Hepatitis C (HCV) from specimens collected by the City of Beaumont Health Department to ensure
that adults in the City of Beaumont are properly diagnosed and treated for these two diseases.
III. TERMS OF AGREEMENT
The City shall obtain patient consent, draw, label, and deliver screens to the Hospital's Laboratory
Department. The Hospital shall perform these screens, in accordance with standard laboratory
practices and regulations. The Hospital shall provide the results in a confidential and secure manner to
the City via secure facsimile. The City shall notify patients of the results and assist these patients, as
needed, with obtaining proper medical care and follow up.
The Hospital shall provide the City the appropriate supplies as needed to draw the specimens. This
may include, but is not limited to: gloves, gauze, hubs, alcohol swabs, needles, tubes, tourniquets, and
tape. The City shall notify the Hospital at least five (5) days in advance with supply requests or
replenishment.
The Hospital shall work with the City to educate these patients identified with HIV -and/or Hepatitis C
about available medical care.
This Memorandum of Agreement may be terminated upon thirty (30) days' notice by either party.
This Memorandum of Agreement shall automatically terminate upon the termination or expiration of
the grant provided by Gilead Sciences to the Hospital related to laboratory screenings of HIV and
Hepatitis C.
