HomeMy WebLinkAboutSOG 203.01J Reviewed 5-2015 Allergic ReactionI. Goal The goal of this policy is to define the standard of care for any patient presenting with an allergic reaction. ll. Protocol A. Institute protocol "203.01A" for all patients. B. Assist with ventilations as needed. C. If patient has a prescription EPI -PEN (auto -injectable epinephrine), you may administer it if the following criteria are met: 1. It is prescribed to the person. 2. The expiration date is current. 3. Patient is in respiratory distress. a. Wheezing b. Tachypnea c. "Tightness" in throat 4. Showing at least one of the following signs: a. Hypotension b. Uticaria/Hives c. Swelling l) Face 2) Hands 3) Feet 4) Tongue 5. Note: When administering medication via an auto -injector device, be certain that there is adequate subcutaneous tissue at the injection site. The auto -injector devices discharge with significant force and can cause injury if utilized at a site with minimal subcutaneous tissue. If the patient has suffered from severe reactions previously but is not displaying signs of anaphylaxis at present, observe very closely for the development of respiratory distress and/or shock and be prepared to rapidly intervene as soon as possible. E. Extreme caution should be used in treating patients that are greater than SS or have a history of heart disease. A conservative approach should be used in treating these patients. The EPI -PEN should only be utilized if allergic reaction symptoms are We threatening. III. Resources A. Epinephrine (Adreline) 1:1,000. PHYSIOLOGICAL ACTIONS: Stimulates sympathetic receptors. THERAPEUTIC EFFECTS: Bronchodilation. Increased systemic vascular resistance. Dilation of coronary arteries. Increased automaticity of myocardium. INDICATIONS: Allergic reaction. CONTRAINDICATIONS: Myocardial ischemia. Hypertension. Tachydysrhythmias. Pulmonary edema. DOSAGE: (Auto -injector administers 0.3 - 0.5 mg) ROUTE: SQ (via auto -injector). SPECIAL NOTES: Causes increased myocardial oxygen demand and increased heart rate. SIDE EFFECTS: Ventricular ectopy, tachydyshythmias. Angina, hypertension, palpitations.
