HomeMy WebLinkAboutIndividual Activity Log Samples INDIVIDUAL ACTIVITY LOG 214i Purpose: The Individual Activity Log records individual detailed tasks and equipment used. These logs provide the basic reference from which to extract information for inclusion in any after-action report. Preparation: The Individual Activity Log is initiated and maintained by each individual. Completed log is to be submitted to his/her immediate supervisor/leader, daily. Distribution: The Documentation Unit maintains a file of all Individual Activity Logs. All completed original forms MUST be given to the Time Unit. Time Monitor Unit will copy and forward the originals to the Finance - Time Unit. Federal Highway This section should be marked if any work is done on a FHWA road. All time and equipment use is eligible and need to be marked accordingly. The time worked and Administration (FWHA) equipment use cannot be combined with time worked on non-FHWA roads and must be documented separately. Item #Item TitleInstructions 1Incident Name Enter the name assigned to the incident. 2Date Enter the date for check-in and check-out. 3Individual Name City Employee: Please use the name that is on the City's payroll 4ICS Section ICS Section of the individual in charge of the Unit. 5Home Agency Home Agency (Department/Division) 6Assignment/Location Enter the name of the Assignment or the Location of Activity. 7Resource Assigned List the start and end times, equipment/vehicle ID number, start mileage and end mileage equipment/vehicle description, and description of activity during the operational period. 8Significant Events Enter the time and description of specific activity/task. Must include CHECK-IN Time and CHECK-OUT Time. These times will function as clocking in and out. 9Employee Signature Enter name and title of the person completing the log (this should be the actual employee). Provide log to immediate supervisor, at the end of each operational period. Date/Time Enter date (month, day, year) and time prepared (24-hour clock). 7 Attention POLICE Department personnel * For Security Detail/Traffic Control point - please document the start and end times; also denote Security Detail or Traffic Control point in the Description of Activity column. * While patrolling, please document the times, start and ending mileage during the patrolling period.
