WHEREAS, on January 8, 2008, the City Council of the City of Beaumont, Texas,
passed Resolution No. 08-005 awarding a contract in the amount of $1,700,059 to T.
Johnson Industries, Inc. for the Proposed New Water Well No. 2 and Related Pilot Hole
Project; and,
WHEREAS, Change Order No. 1, passed by Resolution 08-187 on June 17, 2008,
in the amount of$88,866.14, was required to furnish all labor, materials, and equipment
to provide testing of the well header piping as specified, furnish all labor, materials, and
equipment to provide additional well disinfection,furnish and install, complete in place, one
(1) 24" flanged gate valve for the existing 24" transmission line, and to furnish all labor,
materials, and equipment for cementing between the 26" casing and the 20" liner pipe,
from the top of gravel to ground surface, thereby increasing the contract amount to
$1,788,925.14; and
WHEREAS, Change Order No. 2, dated September 29, 2008, in the amount of
$1,610.00, increased the contract amount to $1,790,535.14; and
WHEREAS, Change Order No. 3, dated May 18, 2009, in the amount of$7,000.00
increased the contract amount to $1,797,535.14; and
WHEREAS, Change Order No. 4 in the amount of$52,638.99 is required for"extra
work" described as deletion of Entergy allowance for electrical service, deletion of aquifer
tests during pilot hole, addition of 80 feet of electrical service from transformer to main
disconnect, addition of electrical components and services for proposed control panels,
addition of fuel transfer pump and associated electrical conduit and conductors, and
furnish and install 500 KW portable generator, thereby increasing the contract amount to
$1,850,174.13;
NOW, THEREFORE, BE IT RESOLVED BY THE
CITY COUNCIL OF THE CITY OF BEAUMONT:
THAT the City Manager be and he is hereby authorized to execute Change Order No. 4
in the amount of$52,638.99 for"extra work"described above, thereby increasing the total
contract amount to $1,850,174.13 for the Proposed New Water Well No. 2 and Related
Pilot Hole Project.
