WHEREAS, on December 16, 2003, the City Council of the City of Beaumont,
Texas, passed Resolution No. 03-271 awarding a contract in the amount of$11,029,000
to Craig Sheffield &Austin, Inc. for the Water Treatment Plant 14 Million Gallons Per Day
(MGD) Expansion and Miscellaneous Improvements Project; and,
WHEREAS, Change Order No. 1, passed by Resolution No. 04-143 on June 29,
2004, in the amount of$1,377,912.01 was required for extra work to install two additional
filter units with all the related appurtenances for a complete operation system, modify the
fill beneath the chemical storage, clean debris in the chemical storage area and change
the existing piping in the chemical building thereby increasing the contract amount to
$12,406,912.01; and,
WHEREAS, Change Order No. 2, passed by Resolution No. 05-003 on January 4,
2005, in the amount of$28,475.60 was required for extra work to add a caustic injection
line at the chemical feed vault, change service pumps to sump pumps, add concrete
sidewalks around the east stairway at Administration Building entry, add pipe support
braces for 60-inch influent settled water piping in filter gallery, and add embedded
waterstop at the filter side of the cold joint required to install the geotextile drainage layer
thereby increasing the contract amount to $12,435,387.61; and,
WHEREAS, Change Order No. 3, passed by Resolution 05-130 on May 24, 2005,
in the amount of$249,396.54 was required to slope pulsator slabs to the drains, install a
concrete plug for the 36-inch drain pipe located at the south clearwell, stabilize fill material
and 27-inch pipe under 72-inch pipe, investigate leak south of the clearwell and fill sink
hole in access road, provide additional hatches and lifting hooks along the filter gallery roof,
provide and install compressed air piping and related appurtenances from the existing
chemical building to the chemical storage area, apply a NSF rated coating on all wet
surfaces at the pulsators and rapid mix basin, provide and install five 30-inch insert
magnetic flow meters at the pulsators instead of five 30-inch flanged magnetic flow meters,
add galvanized steel access ladder at fluoride storage area, provide and install Altivia
chlorine dioxide generators and control revisions, provide additional circuit breakers,
conduits, and wire for pipe heat trace, install Marsh-McBirney insert magnetic flow meter
instead of polysonic flow meter at CF-V2, extend pulsator basin sample piping to the
existing filter gallery, reroute existing 4-inch sanitary sewer force main to the existing
manhole on Pine Street, repair items on the final punch list of the 5 MG Clearwell Project
thereby increasing the contract amount to $12,684,784.15; and,
WHEREAS, Change Order No. 4, passed by Resolution 05-189 on August 30,
2005, in the amount of$90,450.34 was required to dig and locate the leak on the existing
48 inch water transmission line, install a light pole, install two 48 inch butterfly vavles at the
high service pump station discharge connection, install water hose stations, pressure and
caustic taps at the new filters thereby increasing the contract amount to $12,775,234.49;
and,
WHEREAS, Change Order No. 5 is required to modify the medium voltage switch,
relocate chemical taps on the 60-inch filter effluent pipe, install 1-inch sample line from
chemical feed vault to the laboratory, fill storm sewer lines under building with flowable
grout, install 12-inch ductile iron blind flange, perform additional excavation due to paving
elevation revisions, install additional level gauges at the chemical bulk storage tanks,
furnish and install additional concrete paving adjacent to the lagoon, furnish and install
42-inch butterfly valve, furnish and install miscellaneous electrical additions, furnish and
install spare conduits at the new rapid mix basin,furnish and install sun shields on the flow
meters transmitters, furnish and install fiberglass gratings at pulsators 1, 2 & 3, perform
miscellaneous work to decommission the existing filter gallery, replace existing ductile iron
piping in pulsators 1,2&3, perform miscellaneous revisions to piping at chemical building,
furnish and install additional pipe labels needed to meet TCEQ requirements increasing
the contract amount by $178,962.74 thereby increasing the contract amount to
$12,954,197.23.
NOW, THEREFORE, BE IT RESOLVED BY THE
CITY COUNCIL OF THE CITY OF BEAUMONT:
THAT the City Manager be and he is hereby authorized to execute Change Order No. 5
increasing the contract amount by$178,962.74 for a total amount of$12,954,197.23 for
the Water Treatment Plant 14 Million Gallons Per Day (MGD) Expansion and
Miscellaneous Improvements Project.
