BE IT RES(DLA/EE) BY THE CITY COUNCIL
OF THE CITY OF 13EAUMONT:
THAT the City Council hereby approves the purchase of three (3) ambulance
boxes to be mounted to new chassis for use by the Emergency Medical Services (EMS)
[Division from Frazer, Ltd., of Houston, Texas, in the amount of $70E5,684.00 through the
Houston -Galveston Area Council (H-GAC) Cooperative Purchasing Program_
BE IT FURTHER RES(DLA/EE) THAT the City Manager be and he is hereby
authorized to execute all related documents relating to the purchase of the above
described equipment.
The meeting at which this resolution was approved was in all things conducted in
strict compliance with the Texas Open Meetings Act, Texas Government Code, Chapter
551.

Frazer will accept returns on parts up to 180 days after shipment. No restocking fee will be charged if the item is returned within 90 days of the original in voice date. All
parts retums should be shipped back freight prepaid and require prior approval with a Retums Material Authorfza6on' (RMA) clearly displayed on the exterior of the
shipping package. A credit w111 be issued towards the customer's account within approximately 7 business days of receipt of the item. ff a part is returned after 90 days
of the original invoice date a 15% restocking fee will be applied. Frazer L td reserves the right to accept returned items at its sole discretion based upon the condition
of the item to be placed back into stock.
