The goal of this policy is to define the standard of care for any patient presenting with a
complete Texas Department of State Health Services (TDSHS) "Do Not Attempt Resuscitation"
form or approved identification device.
ll. Protocol
A. Honor an "Out of Hospital DNAR" (OOH DNAR) order (No CPR) if:
1. Patient presents with no pulse.
2. Patient presents with a pulse without respirations (excluding airway
obstruction).
3. DNAR order fiom the patient's personal physician is presented at the
scene will be honored in the event of respiratory or cardiac arrest.
B. DO NOT honor an OOH DNAR orders (Initiate CPR) if:
1. Suspicion of suicide, homicide, or other non -natural cause of death.
a. Contact Law Enforcement.
2. Patient is pregnant.
3. Physician issuing the order is not the patient's personal physician.
4. Treat per specific protocol if the patient presents with a pulse and
spontaneous respirations (excluding advanced airway management).
C. In the event, the patient expires during assessment of scene or OOH DNR order
is invoked, the First Responder will:
1. Contact Dispatcher regarding death notification.
2. Request appropriate Law Enforcement agency and Justice of the Peace.
D. In the event the scene situation is unclear or conflicts occurs, the First Responder
will:
1. Begin BLS (CPR) care.

2. Advise the responding EMS unit.
E. Out of State DNAR orders
1. If a DNAR order is presented from another state, territory or possession of
the United States, the senior ECA/EMT will honor the ORIGINAL out of
state DNAR order form with or without the device being present (arm
band, necldace, etc.).
2. The First Responders will honor a COPY of the order ONLY IF the
device is present.
