BE IT RESOLVED BY THE CITY COUNCIL
OF THE CITY OF BEAUMONT:
THAT the City Council hereby approves a three (3)-year contract with Fort Dearborn for
Long Term Disability Insurance in the rates shown below:

Age 0 to 40 $0.25
e 41 to 49 0.62
Age 50 + 0.77
Rate Guarantee 3 years
