BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF BEAUMONT:
THAT the City Manager be and he is hereby authorized to enter into a one (1) year
contract with Moor Employee Benefits for Fortis Benefits for basic life and accidental death
dismemberment and supplemental life and accidental death and dismemberment benefits
with an effective date of January 1, 1995 for the City of Beaumont.
BE IT FURTHER RESOLVED that the City Manager be and he is hereby authorized to
enter into a one (1) year contract with MONY for ITT Hartford for Long Term Disability
insurance for the City of Beaumont.
PASSED BY THE CITY COUNCIL of the City of Beaumont this the /�� day
