WHEREAS, proposals were received for a fully insured Health Maintenance
Organization (HMO) plan for City employees for a period of eighteen months; and,
WHEREAS, CIGNA HealthCare submitted rates as follows:
EMPLOYEE GROUPS MINIMUM PREMIUM PLAN FIXED PREMIUM RATES
Employee Only $136.25 per month $143.94 per month
Employee+ 1 Dependent $328.61 per month $346.98 per month
rEmployee+2 or More $467.87 per month $494.00 per month
WHEREAS, City Council is of the opinion that the minimum premium plan proposal
submitted by CIGNA HealthCare should be accepted;
NOW, THEREFORE, BE IT RESOLVED BY THE
CITY COUNCIL OF THE CITY OF BEAUMONT:
THAT the minimum premium plan proposal submitted by CIGNA HealthCare for a fully
insured Health Maintenance Organization (HMO) plan for a period of eighteen months for
City employees at the above rates be accepted by the City of Beaumont and the City
Manager be and he is hereby authorized to execute a contract with CIGNA HealthCare for
such HMO plan.
PASSED BY THE CITY COUNCIL of the City of Beaumont this the day of
