WHEREAS, on Monday, June 6, 1994,bids were received for an annual contract for
providing medical laboratory services required by the Beaumont Health Department; and,
WHEREAS, Smith Kline-Beecham submitted a bid in the following amounts:

CHEM-ZYME and CBC $ 6.00/ea
Substance Abuse Panel 10 $ 15.00/ea
HIV $ 10.00/ea
Hepatitis B Panel & Surface Antibodies $ 15.75/ea
ABO-RH $ 6.25/ea
Blood and/or Urine Alcohol Testing $ 5.00/ea
Miscellaneous Culture $ 8.00 ea
Discount of all other Laboratory Testing 45%
WHEREAS, City Council is of the opinion that the bids submitted by Smith Kline-
Beecham should be accepted;
NOW, THEREFORE, BE IT RESOLVED BY THE
CITY COUNCIL OF THE CITY OF BEAUMONT:
THAT the bids submitted by Smith Kline-Beecham for an annual contract for providing
medical laboratory services required by the Beaumont Health Department in the above
amounts be accepted by the City of Beaumont. //,
