RESOLUTION
BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF BEAUMONT:
THAT the City Manager be, and he is hereby, authorized to execute a Cooperative Agreement
of Affiliation with Jefferson County regarding provision of health care services, a copy of which
is attached as Exhibit "A".
PASSED BY THE CITY COUNCIL of the City of Beaumont this the / day

-- COOPERATIVE AGREEMENT OF AFFILIATION
This agreement is executed on this the day of
'r 19 between Jefferson County, referred to as the
county in this greement and the City of Beaumont, referred to as
the city in this agreement.
WHEREAS, the city and county both provide health care in a
common area and,
WHEREAS, to bette)r accomplish a common goal of offering non
duplicative health services to all of North Jefferson County
residents, the city and the county agree:
1. The city will provide preventive health services
to residents of North Jefferson County to include
but not limited to child health, WIC, immunizations,
STD, dental., chronic disease screening, HIV counseling
and testing and investigative responsibilities for
vaccine preventable diseases.
2 . The county will provide primary health care for those
individuals 0 - 100 of the Federal Poverty Index;
18 years of age and over. These services .include, but
are not limited to chronic disease treatment and ether
non emergency medical services.
Although both entities will have a shared service area, the
services will be in an nonduplicative manner.
