R E S O L U T I O N
BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF BEAUMONT:
THAT the City Manager be, and he is hereby, authorized
to approve the purchase of reinsurance, life insurance and
accidental death and dismemberment insurance for the City
employees ' group health insurance plan from John Alden Risk
Management Services at the following costs:
Rate per EmployeejMo Rate per Dependent/Mo
Reinsurance Rate $3 .33 $4. 11
Basic Life Insurance $ .23 per $1000 of coverage
Accidental Death &
Dismemberment . $ . 06 per $1000 of coverage
