Various plan revisions (alterations) required during construction, with no "extra work" and utilizing existing bid items. 1 . New manhole MH N-64P was constructed approximately 5 feet west of planned location to keep from obstructing existing drainage ditch on Forrest Street. 2. Line 9-2" from Station 7 + 00 to Station 0 + 02.5 on Ewing Street was relocated from under existing street pavement to behind the street curb and another manhole, MH N-79P, installed. 3. Line "E" (12" force main) was installed at various lower elevations along the route from Station 0 + 38 to Station 6 + 93 and Station 8 + 68 to Station 10 + 54 at the Contractor's request to facilitate ease of construction and possible conflicting utilities. From Station 6 + 93 to Station 8 + 41, the Contractor originally installed this section generally in accordance with the plans; however that extreme pipe joint deflections, due to a poor bore/case installation, resulted in the inability of the force main from holding (passing) the hydrostatic test. Subsequently, the Contractor decided to abandon this section of force main and reinstall a replacement section at a high elevation using the open cut trench method. 4. New manhole MH N-38P was relocated approximately 15 feet east of the planned location in order to intercept the existing 6" sanitary sewer on Stephenson Street. Additionally, the slope of Line "G" (8" sanitary sewer) from Station 0 + 00 to Station 2 + 33 was reduced to .33% in order to intercept the existing 6" sanitary sewer on Stephenson Street. 5. Line "F" (storm sewers) modifications consisting of the addition of two new junction boxes, one new ditch inlet struction, deleting some proposed storm sewer leads, and minor relocations of new storm sewers were constructed to facilitate obstructions from existing utilities, a building's overhanging canopy, and the discovery of previously unknown existing storm sewers. C. To increase the contract amount for "extra work" performed by the Contractor for improvements constructed in addition to the contract plans, as follows: 1 . Additional asphaltic concrete overlay, 2 inches thick, along Miller Street between Grant Avenue and Willow Street, in accordance with the Contractor' s letter dated July 31, 1987: 15.8 S.Y. @ $8.25/S.Y. +$ 130.35 2 . Additional street pavement consisting of reinforced concrete curb and gutter section, approximately 2 feet wide, on Miller Street between Grant Avenue and Willow Street, in accordance with the Contractor's letter dated July 31, 1987: 78 L.F. @ $16.00/L.F. +$ 1,248.00 4of6 3.' Tie"In an existing 12 inch storm sewer lead into the new 21 inch RCP storm sewer using PVC lateral saddle located at Miller Street and Grand Avenue, in accordance with the Contractor's letter dated July 31, 1987: Lump Sum +$ 400.00 4. Clean out and install a new concrete invert inside and existing ditch inlet catch basin which was damaged during boring/casing opera- tions located at the Southwest corner of Forrest Street and Long Avenue, in accordance with the Contractor' s letter dated July 31, 1987: +$ 300.00 Lump Sum 5. The originally proposed conduit run "IP" and wire size for incoming power for the Long Street at Grand Avenue Lift Station was changed from 2-1/2" to 3" conduit and the wire size changed from 3-No. 250 MCM with 1 No. 2 to 3-No. 300 MCM with 1 No. 2, in accordance with the Contractor's letter dated July 31, 1987: Lump Sum +$ 880.00 6. A clean sealant coating was applied to the cement masonary block walls for the Long Street and Grand Avenue Lift Station to prevent moisture penetration , in accordance with the Contractor' s letter dated July 31, 1987: Lump Sum +$ 357 .50 7. A previously unknown 2' x 4' existing storm sewer cast-in-place box culvert under Magnolia Street at Miller Street was unavoidably damaged during the installation of the new 15 inch sanitary sewer. The additional cost to make these cast-in-place repairs and maintain traffic control is in accordance with the Contractor' s letter dated May 31, 1987: Lump Sum +$ 4,830.20 8. The proposed 100 SCFM ventilator for the Long Street at Grand Avenue Lift Station was changed to a 750 SCFM ventilation to conform with State Design Criteria. This additional cost is in accordance with a letter dated September 16, 1987 from the Contractor's lift station subcontractor: Lump Sum +$ 469.20 ITEM C - TOTAL REVISED CONTRACT AMOUNT +$ 8,615.25 D.
