BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF BEAUMONT:
THAT the City Manager be, and he is hereby, authorized to execute the following plan
design and copay changes effective January 1, 1998 to the City's prescription drug
program:
Generic drug $3.00 copay maximum 30-day supply
Name brand with no generic available $15.00 copay maximum 30-day supply
Name brand with generic $20.00 copay maximum 30-day supply
PASSED BY THE CITY COUNCIL of the City of Beaumont this the L day
