THAT the following individuals are authorized as signatories on the Memorial Sisters of
Charity Employee Benefits account number 05007019326 and Dental Claims account
number 05007019318 at Texas Commerce Bank National Association:

THAT the following individuals are authorized to request electronic fund (wire) or ACH
transfers of any and all City funds on deposit with Texas Commerce Bank National
