RESOLUTION NO.
BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF BEAUMONT:
THAT the City Manager be and he is hereby authorized to execute a two (2)-year contract
with Trans Star EMS and Ameristat Mobil Medical Services, Inc. The contracts are
substantially in the form attached hereto as Exhibit "A" and Exhibit "B."

AGREEMENT
WHEREAS, Tran Star EMS Mobil Medical Services, Inc. (herein "Tran Star EMS')
desires to provide emergency medical services as an independent contractor for the City
of Beaumont (herein "City"); and,
WHEREAS, Tran Star EMS will, and in consideration for receiving dispatched
emergency calls from City under the current protocols existing for such dispatch, provide
emergency medical services on a 24 hour basis in the city limits of City;
NOW, THEREFORE, the parties hereto agree as follows:
1. Tran Star EMS agrees to provide emergency medical services in
accordance with the "Emergency Medical Services Standards" attached
hereto as Exhibit "A" and made a part hereof for all purposes.
2. Tran Star EMS and City agree that the term of this Agreement shall
commence upon signing and end April 30, 1999.
3. City agrees to dispatch Tran Star EMS on emergency calls as if it were a
unit of the City of Beaumont Emergency Medical Services System, but as an
independent contractor to the City.
4. This Agreement may be canceled by either City or Tran Star EMS with thirty
(30) days advance written notice.

EXHIBIT "A"
AGREEMENT
WHEREAS, Laidlaw Medical Transportation, Inc. dba Ameristat desires to provide
emergency medical services as an independent contractor for the City of Beaumont
(herein "City"); and,
WHEREAS, Ameristat will, and in consideration for receiving dispatched emergency
calls from City under the current protocols existing for such dispatch, provide emergency
medical services on a 24 hour basis in the city limits of City;
NOW, THEREFORE, the parties hereto agree as follows:
1. Ameristat agrees to provide emergency medical services in accordance with
the "Emergency Medical Services Standards" attached hereto as Exhibit "A"
and made a part hereof for all purposes.
2. Ameristat and City agree that the term of this Agreement shall commence
upon signing and end April 30, 1999.
3. City agrees to dispatch Ameristat on emergency calls as if it were a
