RESOLUTION NO. 2c/-, -213
WHEREAS, on Monday, August 26, 1996, bids were received for an annual contract
for providing medical laboratory services required by the Beaumont Public Health
Department; and,
WHEREAS, Park Medical Laboratory submitted a bid in the following amounts:
DESCRIPTION UNIT COSTS
CBC $ 4.90/ea
CHEM-ZYME and CBC $ 12.50/ea
Substance Abuse Panel 5, DOT split specimen $ 15.00/ea
Substance Abuse Panel 5, Regular $ 15.00/ea
Evidential Breath Testing $ 3.00/ea
HIV $ 7.00/ea
Hepatitis B Surface Antigen $ 5.00/ea
Quantative Hepatitis Bs Ab titer $ 6.30/ea
Hepatitis C Antibody Screen $ 17.55/ea
ABO-RH $ 5.60
Percentage Discount of all other Miscellaneous Tests 65%
WHEREAS, City Council is of the opinion that the bids submitted by Park Medical
Laboratory should be accepted;
NOW, THEREFORE, BE IT RESOLVED BY THE
CITY COUNCIL OF THE CITY OF BEAUMONT:
THAT the bids submitted by Park Medical Laboratory for an annual contract for providing
medical laboratory services required by the Beaumont Public Health Department in the
above amounts be accepted by the City of Beaumont.
PASSED BY THE CITY COUNCIL of the City of Beaumont this the 174 day of
1996.
