HomeMy WebLinkAboutORD 84-130 ORDINANCE NO. ENTITLED AN ORDINANCE ESTABLISHING VARIOUS HEALTH SERVICE FEES; PROVIDING FOR SEVERABILITY; PROVIDING AN EFFECTIVE DATE AND PROVIDING FOR REPEAL. BE IT ORDAINED BY THE CITY OF BEAUMONT: Section 1 . That Chapter 13, Section 16 of the Code of Ordinances of the City of Beaumont is hereby amended to read as follows: Section 13-16: Health Fees The Beaumont Health Department is hereby authorized to charge fees for services in accordance with the following schedule: Activity Fees VD Treatment $10. 00 Duplicate Immunization Cards 3. 00 Health Cards 35. 00 X-rays 15. 00 Duplicate Health Cards 5. 00 Laboratory Water Samples 4. 00 Laboratory Family Planning Tests 2. 00 Laboratory Tests for VD, Jail Inmates 2. 00 Animal Control Pound Fees 15. 00 Animal Control Vaccination Fees 8. 00 Animal Control Boarding Fees 3. 00/per day Animal Control Dead Animal Removal Fee 10. 00/per trip ( 2 animals ) 3. 00/per head Flu Shots 5. 00 Beaumont Community Health Clinic Minimum Medical Fee 2. 00 Beaumont Community Health Clinic Dental Fee 2. 00 Wellness Services to Business 10. 00 basic 13. 00 basic plus screenings Fees for Immunizations, Child Health Screenings, Adult Health Screenings and Vision and Hearing Tests, based upon sworn statements of family income, shall be as follows in accordance with the following schedule: Family Size $1 Fee $3 Fee $5 Fee 1 0 - $ 5 ,478 $ 5,479 - 6 ,972 $ 6 ,973 up 2 7 ,392 7 , 393 - 9 ,408 9 ,409 up 3 9 ,306 9,307 - 11,844 11,845 up 4 11, 220 11, 221 - 14,280 14, 281 up 5 13, 134 13, 135 - 16 ,716 16,717 up 6 15,048 15,049 - 19 ,152 19 ,153 up 7 16,962 16,963 - 21,588 21,589 up 8 18 ,876 18 ,877 - 24,024 24,025 up 9 20,790 20,791 - 26,460 26,461 up 10 22,704 22,705 - 28 ,896 28 ,897 up Section 2. That Chapter 13, Section 17 of the Code of Ordinances of the City of Beaumont is hereby enacted to read as follows: Section 13-17: The fees established in Section 13-16 herein may be waived by the Director of the Department of Health upon proof by the patient that such patient is unable to pay such fee. Such proof may be provided by execution of a sworn statement containing facts sufficient to establish that such patient is unable to pay such fee. Section 3. That Chapter 11, Section 11-3 of the Code of Ordinances of the City of Beaumont be, and the same is hereby, amended to read as follows: Section 11-3; A health registration card shall not be required for persons engaged in serving or handling of food intended or suited for human consumption. Any person may obtain a health registration card from the health department of the city on a voluntary basis after examination by a physician licensed to practice in this state. The applicant shall submit to such examinations, laboratory tests and X-rays as may be necessary to determine if the individual examined is free of any transmissible condition of any contagious 2 - ��4 7 or communicable disease. This shall include but is not necessarily limited to a standard serological test for syphilis, performed in a laboratory approved by the State Depazrtment of Health and an X-ray for tuberculosis of the chest or standard skin test for tuberculosis, followed by an X-ray , if positive, both to be performed within thirty ( 30) days of the application. However, said X-rays as provided for herein shall not be required in any instance in which the health authority or any licensed physician believes said X-ray is not necessary or is dangerous to the health and safety of the applicant. If such person examined is found to be free of any transmissible condition of any contagious or communicable disease, such individual shall be issued a health registration card. If it is necessary for the applicant to have a chest X-ray done by the Health Department, a charge for said X-ray shall be made in accordance with the charges established by Section 13-16 of the Code of Ordinances of the City of Beaumont. If it is not necessary for the individual to have a chest X-ray, or if an X-ray is done by someone other than the health department, a charge for a health card as established by Section 13-16 of the Code of Ordinances of the City of Beaumont shall be made. A duplicate of the health registration card shall be placed in the files of the city health department bearing the same number as the health registration card issued to such individuals. This card may be revoked when, in the opinion of the health authority, the holder is suspected of having a contagious or communicable disease or of being a carier of such; and the health authority reserves the right to make such other examination and tests at any time that may be necessary at the sole direction of the health authority. This registration card and health certificate shall be valid for a period of twelve ( 12) months unless canceled by the health authority as provided by this chapter. A duplicate health card may be obtained by the payment of the fee for a duplicate health card set out in Section 13-16 of the Code of Ordinances of the City of Beaumont. 3 - d e l Section 4 . This ordinance shall be and become effective on November 1 , 1984. Section 5. That if any section, subsection, sentence, clause or phrase of this ordinance, or the application of same to a particular set of persons or circumstances, should for any reason be held to be invalid, such invalidity shall not affect the remaining portions of this ordinance, and to such end the various portions and provisions of this ordinance are declared to be severable. Section 3. That all ordinances or parts of ordinances in conflict herewith are repealed to the extent of the conflict only.
