BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF BEAUMONT:
THAT the City Manager be and he is hereby authorized to enter into a one-year
contract with Blue Cross Blue Shield of Texas commencing January 1, 2003 to
provide third-party administration of the City*s self-funded Preferred Provider
Organization (PPO) medical plan, self-funded dental plan and a fully-insured Health
Maintenance Organization (HMO) with HMO Blue Texas with the option to review
annually at mutually agreeable rates. The plan designs for the PPO and the HMO
shall be as set out in Exhibits "B" and "C" effective January 1, 2003.
PASSED BY THE CITY COUNCIL of the City of Beaumont this the 1st day of
October, 2002.
